About the Role

Join the CO-ACT team at Hampstead Rehabilitation Centre and support older people to achieve the best possible outcomes through person-centred social work. You'll manage complex caseloads, work closely with multidisciplinary teams and help patients transition safely to the next stage of their care.

• Deliver complex psychosocial assessments and interventions
• Support safe discharge and care planning
• Partner with multidisciplinary teams and community services
• Mentor junior staff and social work students
• Drive quality improvement and research initiatives

About You

You're an experienced Social Worker who is passionate about supporting older people through compassionate, evidence-based care.

• Degree eligible for AASW full membership

• Experience managing complex health care caseloads and discharge planning

• Strong assessment, counselling and case management skills

• Ability to work collaboratively across multidisciplinary teams

• Experience providing supervision and clinical guidance
