Social Worker Responsibilities & Duties

• Conduct psychosocial assessments for new residents
• Develop individualized care plans in collaboration with healthcare teams
• Provide individual and group counseling to residents
• Assist residents in adjusting to nursing home life
• Advocate for residents' rights and needs
• Facilitate communication between residents, families, and staff
• Coordinate discharge planning and follow-up services
• Assist families with understanding and navigating the healthcare system
• Organize and implement recreational and therapeutic activities
• Maintain accurate documentation and case records
• Participate in interdisciplinary team meetings
• Offer crisis intervention services as needed
• Provide support and resources for end-of-life care
• Ensure compliance with federal and state regulations
• Conduct family meetings and provide mediation when necessary
• Coordinate with community resources for additional resident support
• Educate staff on psychosocial issues affecting residents
• Monitor and evaluate the effectiveness of care plans
• Develop and implement policies to improve resident care

Social Worker Qualifications & Skills

• Master’s degree in social work (MSW)
• Licensed Clinical Social Worker (LCSW) credentials
• Experience in geriatric or long-term care settings
• Knowledge of Medicare and Medicaid regulations
• Strong communication and interpersonal skills
• Experience with case management and care coordination
