About the Role:

As a Social Worker at Levelland Nursing, you will be responsible for:

• Conducting social assessments and developing individualized plans to meet the unique needs of our patients and residents
• Coordinating with healthcare teams to ensure that patients and residents receive comprehensive care
• Providing emotional support and counseling to patients and their families
• Facilitating communication and collaboration between patients, families, and healthcare providers
• Identifying and addressing social determinants of health that impact patient outcomes

Responsibilities:

• Provide social services to patients and residents, including assessments, counseling, and support
• Develop and implement social plans, including referrals to community resources and services
• Coordinate with healthcare teams to ensure that patients and residents receive comprehensive care

• Maintain accurate and timely records of patient interactions and services provided

Requirements:
