Social Worker

Overview

We are seeking a compassionate and dedicated Social Worker to provide essential support and services to individuals and families in need. The ideal candidate will assist clients in accessing, applying for, and managing various State and Federal benefit programs. This role involves working closely with residents to help them navigate resources. A strong focus on case management for senior citizens and the elderly is essential to ensure comprehensive support tailored to their specific needs.

Responsibilities

• Provide guidance and assistance to clients in accessing and applying for benefit programs such as SNAP, Medicare/Husky-Medicaid, the Medicare Savings Program, Renters Rebate Tax Relief, and CEAP.
• Collaborate with utility companies like Eversource to help residents set up affordable payment plans.
• Conduct assessments to determine individual needs, especially focusing on senior citizens and elderly populations.
• Advocate for clients by providing information about available resources and assisting with paperwork or procedural requirements.
• Document case progress accurately while maintaining confidentiality and compliance with organizational policies.

Qualifications

• Experience in case management or social work roles, particularly working with vulnerable populations such as seniors or low-income families.
• Strong knowledge of local resources, grants, and community organizations serving residents’ needs.
• Excellent communication skills with the ability to advocate effectively on behalf of clients.
• Ability to work independently while managing multiple cases efficiently.
• Compassionate approach with a focus on client-centered support tailored to individual circumstances.

Pay: $24,000.00 per year
