Contract: 13 weeks of contract

Shift Option:

• 37699962 - Mid & Night - shift 11am - 7:30pm - 7:00pm - 3:30am - 8hrs
• 37711918 - Mon - Fri 8am - 4:30pm - 8hrs
• Varied - Day or nights -8hrs

Pay Rate:

• Local: $65/hr
• Travel: $2650/Weekly

Job Summary:

Responsible for providing social services to adult and pediatric patients and their families, including psychosocial assessments, therapeutic interventions, advocacy, education, community resource referrals, and discharge planning support. The role also provides case coverage and support for other Clinical Social Work and Care Coordination staff and participates in teaching and supervision activities.

Key Duties & Responsibilities

Conduct inpatient and outpatient psychosocial assessments for adult and pediatric patients and families.

Provide therapeutic interventions, advocacy, education, and community resource referrals.

Develop and implement psychosocial interventions supporting adherence to medical, surgical, and pharmacological treatment plans.

Develop, implement, review, and update age- and disease-appropriate treatment and care plans.

Maintain accurate and timely daily progress notes and document incidents or unusual occurrences.

Collaborate with patients, families, physicians, and interdisciplinary teams to coordinate care and facilitate appropriate transfers.

Coordinate multidisciplinary referrals and facilitate access to required services.

Participate in treatment planning meetings to review patient needs, progress, and discharge plans.

Support patients and families in participating actively in their care planning.

Requirement:

• 1 year of Social Worker Exp
• ASCW or LCSW
• Master’s degree in social Worker
• Palliative Care experience required.
• Traveling/contract experience required — minimum of 1 previous travel contract.
