Social Worker

1099 Social Worker (LLMSW/LMSW) – Maternal Infant Health Program (MIHP)

Premium Care MIHP LLC is seeking a compassionate, dependable, and motivated Licensed Social Worker (LLMSW or LMSW) to join our growing Maternal Infant Health Program (MIHP) as an Independent Contractor (1099). This position offers a flexible schedule while providing meaningful support to pregnant women, postpartum mothers, and infants throughout Wayne and Oakland Counties.

Compensation

This is a 1099 independent contractor position. Compensation is based on the approved service provided and is processed after all required visit documentation has been completed, submitted, and approved in accordance with MIHP and agency requirements.

Responsibilities

• Conduct home visits with pregnant women, postpartum mothers, and infants.
• Complete psychosocial assessments and MIHP Risk Identifier assessments as assigned.
• Assess emotional well-being, support systems, safety concerns, housing, transportation, and other psychosocial needs.
• Provide education, support, care coordination, and community resource referrals.
• Assist beneficiaries in accessing services such as WIC, SNAP, housing, transportation, behavioral health, and other community resources.
• Collaborate with the Registered Nurse to develop and implement individualized Plans of Care.
• Complete Safety Plans and MIHP interventions when indicated.
• Maintain communication with healthcare providers, community agencies, and referral sources as appropriate.
• Complete progress notes, contact logs, communication forms, and all required MIHP documentation within 72 hours of service delivery.
• Participate in case conferences and team meetings as needed.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program.
• Current Michigan LLMSW or LMSW license in good standing.
• Valid Michigan driver's license.
• Reliable transportation and willingness to travel throughout Wayne and Oakland Counties.
• Strong communication, organizational, and documentation skills.
• Home visiting, maternal-child health, public health, case management, or community-based experience is preferred but not required.
