The role also includes supporting patients within the older persons mental health service, working with patients and their families around psychosocial needs, discharge planning, care coordination and transition back into the community.
You’ll work closely with patients, families and carers and be an integral part of a diverse and collaborative MDT including Physiotherapists, Occupational Therapists, Speech Pathologists, Dietitians and Allied Health Assistants.
Mandatory qualifications and experience:
- Holds a Masters degree in Social Work
- Current (less than 3 months) National Criminal History check (or agreement to obtain prior to commencing employment)
- Experience working in a rehabilitation and/or older persons mental health setting
- Confident supporting patients and families through complex situations and transitions in care
- Strong communication, advocacy and problem-solving skills
- Enjoys collaborative multidisciplinary working
- Able to work independently while also working closely with other Social Workers and wider MDT
We offer:
- a friendly and supportive environment within a multidisciplinary team

- opportunity to be part of an experienced team who provide support and are open to new ideas and growth within our service
The position is casual, with approximately 1–2 shifts per week. Shifts are likely to be Mondays and Fridays, however this is negotiable for the right candidate.
This is a great opportunity for a Social Worker looking for flexible work, a varied caseload and the support of an established multidisciplinary team, with the opportunity to work across both rehabilitation and older persons mental health.
If you’re an enthusiastic Social Worker who enjoys working collaboratively and making a meaningful difference to patients and their families, we’d love to hear from you!
