Help someone feel heard. Help a family find answers. Help a resident take the next step toward home.

The Medical Suites at Oak Creek is seeking a compassionate, motivated Social Worker to make a meaningful difference in our skilled nursing and rehabilitation community in Oak Creek, Wisconsin.

In this role, you will support residents and families through important transitions, advocate for individual needs, and help coordinate the services that make recovery and everyday life better. If you enjoy building relationships, solving problems, and seeing the impact of your work, we would love to meet you!

Why The Medical Suites at Oak Creek?

Our community provides skilled nursing, short-term rehabilitation, and long-term care, giving you the opportunity to support residents with a variety of goals and needs.

  • See your impact. Help rehabilitation patients prepare for a safe return home while supporting quality of life for residents receiving ongoing care.
  • Collaborate across disciplines. Work alongside nursing, rehabilitation, medical providers, and other care team members to support the whole person.
  • Put your skills to work. Our specialty programs include orthopedic rehabilitation, stroke recovery, cardiac care, pulmonary rehabilitation, and palliative care.
  • Build meaningful connections. Get to know residents and families, understand their priorities, and help ensure their voices are part of the care plan.
  • Support life beyond medical care. Our community offers activities, social gatherings, pet visits, a courtyard, a community garden, and spaces for family visits.

What You Will Do

  • Complete social histories and psychosocial assessments to identify residents’ strengths, preferences, support systems, and needs.
  • Develop and update individualized social services care plans with the interdisciplinary team.
  • Coordinate discharge planning beginning at admission, including referrals to community resources, home health services, and other appropriate supports.
  • Provide emotional support and help residents and families adjust to admission, changes in health, and transitions in care.
  • Participate in care conferences and communicate with residents, families, representatives, and healthcare partners.
  • Advocate for resident rights, dignity, choice, and person-centered care.
  • Assist with advance care planning resources, appropriate guardianship referrals, and access to financial or community assistance.
  • Complete timely, accurate documentation and assigned Minimum Data Set (MDS) assessments.
  • Help address resident concerns and connect individuals with appropriate behavioral health and other specialized services.
  • Support staff education on the social and emotional needs of residents.

What We Are Looking For

  • Current Wisconsin social work certification or licensure in good standing, such as CSW, APSW, CISW, or LCSW.
  • Bachelor’s degree in social work or a related human services field, with education meeting Wisconsin requirements for the credential held.
  • Experience in skilled nursing, long-term care, rehabilitation, hospital social work, or another healthcare setting preferred.
  • Knowledge of discharge planning, care coordination, resident rights, and community resources.
  • Familiarity with MDS, Medicare, Medicaid, and nursing home regulations preferred.
  • Strong communication, organization, documentation, and problem-solving skills.
  • A compassionate approach and a commitment to helping residents maintain their dignity, independence, and quality of life.
