Position Qualifications:

LSW required

Acute inpatient experience preferred

In-depth knowledge of psychotherapy methods and available community resources

Understanding of resiliency and recovery in mental illness and substance abuse, and treatment approaches and techniques.

Key Responsibilities / Job Accountabilities:

• Ensures completion of individual, family, and/or group assessments.
• Familiar with outpatient setting.
• Provide short term crisis intervention, as needed.
• Assist patients and families with community resources for post-hospital needs.
• Monitor patient progress toward discharge, ensuring completion of requirements within expected length of stay.
• Assist patients and families with securing assistance services, as needed, e.g. Medicaid applications.
• Facilitates or assist with psychotherapy groups.
• Provide individual counseling.
• Maintain an efficient, organized work environment.
• Maximizes office productivity through proficient use of appropriate software applications.

• Coordinates services with other providers, partners, and stakeholders.
