BayWest Medical Centre – Ajax, Ontario

BayWest Medical Centre is seeking a Registered Social Worker to join our growing healthcare team. The successful candidate will provide compassionate, patient-centred support to individuals and families experiencing a range of social, emotional, and life challenges.

Position responsibilities

• Conduct psychosocial assessments and identify patients’ social and emotional needs
• Provide individual counselling and supportive interventions
• Support patients experiencing stress, anxiety, grief, family concerns, and other psychosocial challenges
• Assist patients with navigating community resources and social services
• Provide support with housing, financial, employment, and other social needs where appropriate
• Develop individualized care plans and provide appropriate follow-up
• Collaborate with physicians and other healthcare professionals as part of a multidisciplinary team
• Provide crisis support and appropriate referrals when required
• Maintain accurate and timely documentation in the EMR
• Maintain patient confidentiality and adhere to professional and ethical standards

Qualifications

• Registered Social Worker in good standing with the Ontario regulatory body
• Bachelor’s or Master’s degree in Social Work
• Previous experience in a healthcare, primary care, or community setting is an asset
• Strong counselling, communication, and interpersonal skills
• Ability to work independently and collaboratively within a multidisciplinary healthcare team
• Experience with electronic medical records (EMR) is an asset
• Compassionate, professional, reliable, and patient-focused
