Social Worker MSW

Are you a dedicated social worker ready to provide vital, frontline advocacy for patients across the healthcare continuum? At RWJBarnabas Health, our Case Management teams work collaboratively to support patients and families navigating the social, financial, and psychological impacts of illness. As an essential member of our interdisciplinary teams, you will partner directly with physicians, nurses, and allied health professionals to ensure the emotional and social aspects of patient care are championed. We view our social workers as a critical component of the care plan, redefining the patient experience through compassion, resource navigation, and teamwork. We hope you are excited by the opportunity to deliver impactful care that empowers our patients and communities!

As a Social Worker MSW, a typical day might include the following:

  • Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient’s unique needs.
  • Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and care transitions.
  • Connecting patients with critical community resources, medical equipment, or post-acute care, and conducting diligent follow-ups.
  • Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and advocate for patient needs.
  • Coordinating social work services within the palliative care team and facilitating patient-family meetings for goal setting and advanced care planning.
  • Serving as a liaison between hospital clinical teams, quality improvement, community hospices, home care agencies, and long-term care facilities.
  • Entering accurate, clear, and timely clinical documentation and intervention records into the patient’s electronic medical chart.

This Role Might Be For You If:

  • You are passionate about helping patients and families resolve complex social, financial, and emotional barriers to health and recovery.
  • You excel in collaborative environments and take pride in building strong, supportive working relationships with multidisciplinary care teams.
  • You possess excellent communication, writing, and presentation skills, maintaining a high bar for timely clinical record-keeping.
  • You enjoy continuous learning and are eager to attend educational seminars to bring new community resources back to your team.
  • You believe that true patient healing and successful transitions are optimized when medical expertise meets deep empathy and advocacy.

To be considered for this opportunity, you must have:
