Social Worker MSW - PACE

Role Overview: Provides social work services within an interdisciplinary team, focusing on direct participant support through counseling, assessment, and resource linkage to promote independence and well-being of older adults.

Key Responsibilities

• Conducts social work assessments and interventions in various settings, including homes.
• Develops individualized care plans considering social, emotional, cognitive, and cultural factors.

• Engages in one-on-one and group counseling, collaborating with team members to address social issues.

Qualifications & Skills

• Master's Degree in Social Work.
• Washington Clinical Independent Social Worker License or eligible temporary/license upon hire.
• Minimum of 2 years' social work experience with geriatric populations from diverse backgrounds.
• At least 1 year of experience working specifically with older adults.
