Pay: Starting at $40/hr+ DOE & licensing, with travel stipends up to $125/day for nearby travel.
Responsibilities include: Conducting psychosocial assessments, collaborating on care plans, providing crisis support, grief counseling, and assisting with legal, financial, and end-of-life planning. Participate in weekly interdisciplinary team meetings and maintain compliant documentation.
Qualifications: MSW required; Utah LCSW preferred; experience in hospice or home health is a plus; valid driver’s license and background check required.
