Social Worker/Case Management

Overview

We are looking for a social worker and/or case manager to join our team in Wavecrest Assisted Living.

Responsibilities

• Conduct thorough patient assessments to determine individual needs
• Coordinate with healthcare providers, community organizations, and families to facilitate discharge planning and ongoing support.
• Manage medical documentation accurately while adhering to HIPAA regulations and social work practice standards.
• Engage in crisis intervention and provide immediate support for patients.
• Assist with intake forms and new resident charts.
• Initial, 30 day and annual assessments.
• Speak with the residents on a daily basis to address concerns and needs.
• Record case notes on each resident and work closely with the administrator and nurse

Qualifications

• Bachelor's degree in social work, psychology, or a related field
• Proven experience working with diverse populations including geriatrics, individuals with disabilities, and/or the elderly.
• Strong skills in patient communication, motivational interviewing, and crisis management.
• Experience with assisted living is a plus
• Ability to work collaboratively within an interprofessional team while maintaining confidentiality and adhering to HIPAA standards.
