Spanish-Speaking Social Worker – Adult Day Health Care (ADHC)

We are seeking a motivated, compassionate, and organized Spanish-speaking Social Worker to join our Adult Day Health Care (ADHC) team. This is a great opportunity for someone who enjoys working with people and is interested in developing their skills in a supportive healthcare environment.

Qualifications

• Bachelor’s degree required
• Must be fluent in Spanish and English
• Previous social work or ADHC experience is not required
• Must be a fast learner and willing to be trained
• Strong communication and interpersonal skills
• Comfortable working with older adults and individuals with various healthcare and social needs
• Organized and able to complete documentation and paperwork accurately
• Basic computer skills
• Dependable, professional, and able to work as part of a multidisciplinary team

Responsibilities

• Assist participants with social, emotional, and community-resource needs
• Communicate with Spanish-speaking participants and their families/caregivers
• Complete and maintain required ADHC social work documentation and paperwork
• Assist with assessments, care plans, progress notes, and other participant records as assigned
• Help connect participants and families with appropriate community resources and services
• Communicate and coordinate with nursing, activities, administration, and other members of the interdisciplinary team
• Assist participants on the center floor when needed
• Maintain confidentiality and follow HIPAA and ADHC policies
• Learn and follow ADHC documentation requirements and center procedures
• Perform additional social service and administrative duties as needed

We are looking for someone who is friendly, responsible, patient, organized, and eager to learn. Experience is not mandatory—we are willing to train the right candidate who is motivated and comfortable working directly with people as well as completing paperwork.
