Speech-Language Pathologist (SLP) – Skilled Nursing Facility

Location: Petaluma, CA

Schedule: Full-Time, Part-Time, or PRN

We are seeking a Speech-Language Pathologist (SLP) to join a supportive therapy team in a Skilled Nursing Facility in Petaluma, CA . This is a great opportunity for an SLP looking for flexibility, a collaborative work environment, and the opportunity to make a meaningful difference in the lives of adult and geriatric patients.

Full-time, part-time, and PRN schedules are available.

Responsibilities

• Evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders
• Develop individualized treatment plans based on patient needs and goals
• Provide dysphagia assessment and treatment
• Collaborate with PT, OT, nursing, physicians, and other members of the interdisciplinary team
• Educate patients, families, and caregivers regarding treatment and strategies
• Complete accurate and timely clinical documentation
• Participate in care planning and discharge recommendations

Qualifications

• Current California Speech-Language Pathologist license
• Master’s degree in Speech-Language Pathology
• ASHA CCC-SLP preferred
• Skilled Nursing, rehab, or geriatric experience preferred, but qualified candidates from other settings are encouraged to apply
• Strong communication and interdisciplinary teamwork skills
