Position Overview

As a Home Health Speech Therapist / Speech-Language Pathologist, you will evaluate and treat patients in their home environment who are experiencing difficulties with communication, cognition, swallowing, or related conditions.

You will develop individualized treatment plans based on each patient’s needs and goals and work closely with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary care team to help patients improve their safety, independence, communication, and overall quality of life.

Key Responsibilities

• Complete comprehensive speech therapy evaluations and develop individualized plans of care and treatment goals.
• Provide skilled speech-language pathology services in accordance with physician orders, the patient’s plan of care, and agency standards.
• Evaluate and treat communication, speech, language, cognitive, and swallowing disorders as appropriate to the patient’s needs.
• Monitor patient progress and modify treatment plans as needed in collaboration with the physician and interdisciplinary care team.
• Educate patients, caregivers, and family members on strategies and techniques to support communication, swallowing safety, cognition, and independence in the home.
• Coordinate care and communicate effectively with physicians, nurses, therapists, office staff, and the Clinical Supervisor.
• Identify changes in patient condition and communicate concerns or recommendations to the appropriate members of the care team.
• Maintain accurate, complete, and timely clinical documentation.
• Participate in case conferences, team meetings, and ongoing education and training.
• Provide guidance and support to other members of the care team when appropriate.

What We’re Looking For

We’re looking for a Speech Therapist who is compassionate, dependable, and comfortable working independently while still being part of a supportive interdisciplinary team.

The ideal candidate has strong communication and teaching skills, sound clinical judgment, and a patient-centered approach to care. Home health experience is welcome, but it is not required — we are happy to train the right candidate.

Qualifications & Certifications

• Graduate of an accredited Speech-Language Pathology program.
• Current Speech-Language Pathologist license in the State of Colorado.
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP), as applicable.
• Current CPR certification.
• Current driver’s license and reliable transportation.

Job Types: Full-time, PRN

Pay: $75.00 - $80.00 per hour
