Sr Nurse Practitioner

Job Requirements

The University of Maryland Medical Center is seeking a compassionate and collaborative Nurse Practitioner to join our inpatient Palliative Care team within a high-acuity academic medical center environment.

This position offers the opportunity to work alongside an interdisciplinary team providing specialized care to patients with serious and complex illnesses. The Nurse Practitioner will support symptom management, goals-of-care discussions, advance care planning, and patient/family support while collaborating closely with physicians, nursing staff, social work, chaplaincy, and consulting services.

Our palliative care team plays a vital role in delivering patient-centered, evidence-based care across a diverse inpatient population within one of the region’s leading academic health systems.

Work Experience

Key Responsibilities

• Provide inpatient palliative care consultations and follow-up care
• Provide compassionate and sensitive support and communication with patients, families, and health care teams regarding terminal illness and disease processes
• In-depth understanding of palliative care principles, hospice philosophy, hospice eligibility and end of life care
• Conduct comprehensive patient assessments and develop treatment plans
• Assist with symptom management and pain management strategies
• Facilitate goals-of-care and advance care planning discussions
• Collaborate with multidisciplinary teams to optimize patient outcomes
• Support patients and families through complex medical decision-making
• Maintain accurate and timely documentation within the electronic medical record
• Knowledgeable on palliative and hospice quality measures
• Serves as a mentor to assist other APPs to develop expertise in clinical skills, decision making, diagnostic reasoning and problem solving
• Mentors others in leading development and implementation of evidenced based best practice.
• Serves as interdisciplinary team leader or committee chair who can lead initiatives to improve patient outcomes inside UMMC as well as outside of the organization
• Takes the lead in difficult situational decision making, viewed as a resource for other healthcare team members
• Leads or mentors others in leading quality improvement initiatives in area of practice.
• Serves as a role model educator for nurses and other healthcare personnel.

Qualifications

• Master’s or Doctoral degree from an accredited Nurse Practitioner program
• Nurse Practitioner licensed in the state of Maryland
• National board certification required
• DEA and CDS registration preferred or eligible
• Prior inpatient palliative care and hospice experience required
• Minimum 3 years nurse practitioner experience required; must have at least 2 years of palliative experience
• Strong communication and interdisciplinary collaboration skills
