Key responsibilities include dispensing and verifying prescriptions, performing drug utilization reviews (DUR), collaborating with healthcare teams to optimize medication regimens, monitoring for drug interactions and adverse reactions, ensuring regulatory compliance, counseling staff on medication safety, and maintaining accurate records.
Qualifications & Skills
- Active California Pharmacist license (RPh) in good standing
- Doctor of Pharmacy (PharmD) from an accredited institution
- Experience in long-term care, skilled nursing, or institutional pharmacy preferred
- Strong knowledge of geriatric pharmacotherapy and medication management
- Familiarity with California Board of Pharmacy regulations and OBRA guidelines
- Excellent attention to detail, organizational, and communication skills
