What they need
For a new client, the intake process is their first impression of Kanda. You’re the person who makes it a good one.
As a Support at Home Inflow Coordinator, you’ll manage the journey from referral through to service commencement — keeping things moving, making sure nothing gets missed and ensuring every client and family feels supported from the very first contact. It’s fast-paced, detail-driven work that sits at the front door of our Support at Home program, and when you get it right, clients arrive into care feeling confident rather than overwhelmed.
What you’ll be doing
- Receiving, triaging and processing referrals from My Aged Care, hospitals and community organisations — reviewing for completeness and flagging immediate risks
- Contacting prospective clients within required timeframes and commencing onboarding activities
- Facilitating efficient client onboarding — consents, documentation and accurate record establishment
- Supporting clients and families to navigate funding arrangements and service commencement
- Building relationships with My Aged Care, hospitals, discharge planners and community referral partners
- Working with rostering and coordination teams to match clients to available workforce and coverage
- Tracking referral conversion, onboarding timelines and pipeline activity to drive continuous improvement
You’re the right person if...
- You have experience in intake, admissions, referrals, care coordination or aged care administration
- You have knowledge of My Aged Care pathways, funding programs and the Support at Home program
- You build positive relationships and communicate clearly in writing and on the phone
- You’re highly organised and able to manage multiple referrals and competing priorities without things slipping
- You’re confident using electronic client management systems
- You hold a current Drivers Licence, Working with Children Check & National Police Check (or you are willing to obtain)
