Support coordinator

Exciting opportunity in Western Sydney!

Your new company is a growing organisation in Western Sydney looking to hire a support worker on an ongoing basis.

Your new role will include but will not be limited to:

• Support the NDIS participant to:
• Assess a number of mainstream, community, informal and provider options
• Develop service agreements and create service bookings with preferred provider.
• Arrange for any assessments required to determine the nature and type of funding required.
• Determine the budget for each support type and advise any relevant plan manager about the breakdown of funds.
• Participate in continuous improvement, ensure prompt action and work with management staff in formulating and administering corrective action plans.
• Effectively communicate and maintain relationships with the NDIA.
• Remain up to date with the disabilities-related policies and procedures in accordance with federal and state regulations.
• Maintain confidentiality of customers, their families/carers and the organisation.

What you'll need to succeed

• Previous experience in disability services, community services, case coordination or a related field.
• Understanding of the NDIS and person-centred support.
• Excellent communication and interpersonal skills.
• Strong organisational and time-management skills.
• Ability to coordinate multiple participants, staff and competing priorities.
• Working with children check and police check (or willingness to obtain one)
