What they need

Make a Difference and Create Opportunities

If you are looking for an opportunity to create meaningful change in an organisation helping to shape local communities, contact Fair Foundations. With a history spanning more than 50 years, our Mission is “Creating strong communities and fulfilling lives by disrupting the cycle of disadvantage in Fairfield and Greater Western Sydney”. Our services include community development, case work, psychosocial and educational programs, disability services and children’s services.

About this opportunity:

Reporting to the Head of Disability Services, the Support Coordinator provides person-centred, trauma-informed capacity-building support to NDIS participants, including people with complex needs. You will help participants understand and implement their plans, address barriers, connect with mainstream, community and provider supports, and build confidence to coordinate and manage their services.

Key responsibilities include supporting participants to prepare for plan reviews, arranging assessments, coordinating service agreements and funding allocations, maintaining accurate case notes and reporting, building stakeholder relationships, and developing a sustainable Support Coordination caseload that meets agreed targets.

What we offer:

  • Generous salary packaging of up to $18,500 per annum
  • 17.5% leave loading
  • Two gifted days per annum, pro rata for part-time employees
  • Employee Assistance Program providing free counselling and mental health support
  • Discounts and perks
  • Career development and training
  • 40% discount on out-of-pocket childcare fees

About you:

You bring a relevant Certificate IV, Diploma or Advanced Diploma in Disability, Community Services, Mental Health, Welfare, Mental Health Peer Work or Counselling, together with at least three years’ experience providing support coordination to people with complex needs. Understanding psychosocial support needs and ability to provide practical and recovery- oriented support for people is highly regarded.

You have sound knowledge of the NDIS Quality Standards and principles, experience implementing participant plans, and the ability to build effective relationships with participants, families, providers and community stakeholders.

You will also have high-level communication skills, experience working with culturally and linguistically diverse communities, current First Aid and CPR certificates, a valid Class C NSW Driver Licence with access to a vehicle, and a current NDIS Worker Screening Check. A current Working with Children Check, or willingness to obtain one promptly, is also required.

The closing date for applications is Wednesday, 28 October 2026 at 11.59pm.

Please refer to the Position Description on our website for the full selection criteria. Please apply through the link.

Applicants should include a cover letter addressing the attributes and skills listed in the Position Description with a current resume. Only applications with both cover letter and resume will be considered.

We review applications regularly so don’t hesitate to apply. However, we only contact applicants selected for interview.

Our workforce is as diverse as the communities we serve, and we encourage First Nations people as well as people from culturally and linguistically diverse backgrounds, of any faith or gender identity to apply. All Fair Foundations positions are subject to a Working with Children Check (WWCC) and a National Disability Insurance Scheme Worker Screening Check (NDISWC). Successful applicants will be required to complete the ‘National Child Safety Training’ provided by the Australian Government Department of Education prior to commencing in the role. This training can be accessed via the Departments learning portal https://learning.education.gov.au/user\_login
