This is a full-time position, working Monday to Friday from 9:00am to 5:00pm.
About the role
As a Support Coordinator with YVSC, you will manage your own caseload of NDIS participants while working closely with participants, families, nominees, providers and other professionals.
Your day-to-day responsibilities will include:
- Building strong, professional relationships with participants and their support networks
- Helping participants understand and implement their NDIS plans
- Connecting participants with suitable providers and community supports
- Coordinating services and maintaining communication between key stakeholders
- Monitoring plan utilisation, budgets and service delivery
- Identifying and responding to gaps, risks or barriers within a participant's support arrangements
- Supporting participants through plan reassessments, changes in circumstances and NDIS processes
- Preparing progress reports, supporting documentation and other correspondence for the NDIA
- Maintaining clear, accurate and timely case notes and participant records
- Working collaboratively with our Support Coordination, Operations and Administration teams
About you
We are looking for someone who is approachable, professional and confident managing a busy caseload while keeping participants at the centre of their work.
You will ideally have:
- Previous experience in NDIS Support Coordination or a closely related role
- A strong understanding of the NDIS and participant plans
- Excellent written and verbal communication skills
- Excellent time-management and organisational skills, with the ability to manage competing priorities, meet deadlines and maintain oversight of a busy caseload
- Confidence communicating with participants, families, providers and external stakeholders
- The ability to work independently while also contributing to a supportive team environment
- Strong problem-solving skills and the ability to remain calm when navigating complex situations
- Strong attention to detail and a commitment to accurate, timely documentation
- Confidence using online systems and case-management software
Experience working with participants with complex support needs will be highly regarded.
What you'll need
The successful applicant will be required to hold or obtain:
- NDIS Worker Screening Check
- Working with Children Check, where applicable
- Current driver's licence
- Relevant qualifications and/or demonstrated experience within disability, community services, allied health or a related field
Why join YVSC?
At YVSC, Support Coordinators are part of a team — not left to manage complex situations alone.
You'll work alongside an experienced and approachable team across Support Coordination, Operations and Administration, with people around you to collaborate with, problem-solve with and draw on when things get complicated.
We value open communication, practical support and strong relationships, both within our team and with the participants we support.
Our Support Coordinators are trusted to manage their own caseloads and work independently, while still having the support of a team around them when they need it.
We want someone who takes ownership of their work, communicates well, stays organised and takes pride in providing reliable and responsive support to participants.
What we offer
- Full-time, Monday–Friday hours
- Consistent 9:00am–5:00pm working hours
- Lilydale office location
- Hybrid working arrangements once established in the role
- An established and growing participant base
- Supportive and approachable management
- A collaborative team environment where you can ask questions and seek support
- Opportunity to grow and develop within an established organisation
