Role: Direct Support Professional

Responsibilities

• Provide person-centered care to individuals with disabilities in home and community settings
• Assist with daily living tasks, personal care, and transportation
• Support community inclusion and engagement in meaningful activities
• Follow individualized care plans, document services, and collaborate with families and care teams
