Support Worker/ Carer

Overview:
We are seeking compassionate and dedicated Support Workers to join our team. As a Support Worker, you will provide essential care and assistance to individuals in need. This role requires strong interpersonal skills, a caring nature, and the ability to work effectively in a team environment.

Duties:

• Provide personal care and support to individuals in their daily activities
• Assist with mobility and transfers, including heavy lifting when necessary
• Administer medication and monitor health conditions as required
• Implement care plans and ensure individual needs are met
• Provide emotional support and companionship to individuals
• Assist with meal preparation and feeding if necessary
• Support individuals with dementia or behavioral challenges
• Collaborate with other healthcare professionals to ensure holistic care
• Maintain accurate documentation of care provided

Skills:

• Experience in senior care, patient care, or dementia care is highly desirable
• Valid driver's license and reliable transportation for home visits if required
• Knowledge of Person Centered care and approaches
• Ability to manage challenging behaviors with patience and empathy
• Strong communication skills, both verbal and written
• Ability to follow care plans and adapt to individual needs
• Knowledge of health and safety regulations in a health and social care settings
