Are you warm, reliable and motivated, with a genuine passion for helping someone live life as independently, safely and meaningfully as possible? This is a unique and rewarding opportunity to provide person-centred support at home and in the community, helping someone take part in the activities they enjoy and continue developing their independence, confidence and wellbeing.

About the role

You will provide practical, personal and emotional support with daily living, personal care, health and therapy routines, communication development, leisure activities and learning opportunities. The role involves working closely with a small support team, the Team Leader, Case Manager and relevant professionals to deliver consistent, high-quality care.

The person you will support enjoys a busy and sociable life, including music, sailing, boccia and creative writing. You will play an important part in enabling them to access the community, enjoy chosen activities and make the most of everyday opportunities.

Key responsibilities

· Provide respectful personal care, including washing, bathing, toileting, dressing, eating and night care.

· Support access to the local community and participation in chosen activities.

· Assist with health, therapy and development programmes under professional guidance.

· Support communication development and the use of computers and assistive technology.

· Administer medication as prescribed and maintain accurate records.

· Prepare meals, snacks and drinks and support day-to-day domestic tasks connected to care.

· Drive the client in their car or another suitable vehicle, subject to assessment and insurance.

· Maintain a safe, positive environment at home and in the community.

· Complete daily reports and communicate clearly with the Team Leader about progress, concerns or incidents.

· Respect confidentiality, privacy and family life at all times.

About you

We are looking for bright, energetic and empathetic individuals who can bring patience, creativity and positivity to the role. You will be dependable, well organised and confident communicating with the client, family members, colleagues and professionals.

Essential skills and qualities

· A genuine commitment to delivering high-quality, person-centred care.

· Experience supporting dependent clients.

· Excellent communication, organisation and record-keeping skills.

· Ability to work flexibly across a range of shifts, sometimes at short notice.

· Confidence working independently while seeking support when needed.

· Ability to work closely with professionals as part of a multidisciplinary team.

· Competent driving skills and a valid driving licence.

· Good IT skills, including Word and Excel.

Desirable

· Experience supporting neurologically impaired clients.

· Experience working with clients in a community setting.

· A willingness to develop your skills and career.

· Ability to swim.

· Ability to accompany the client on holiday if required.

What we offer

· A meaningful and rewarding role where you can make a real difference.

· The opportunity to support an active, sociable person with varied interests.

· Training, guidance and support from the Team Leader, Case Manager and wider professional team.

· The chance to work as part of a small, committed and supportive team.
