Targeted Case Manager

Overview

Targeted Case Managers help families navigate complex systems and develop a unique individualized support plan. They are advocates as well as resources for connecting families with the tools they need to be self-empowered.

ABA-LLC seeks qualified, compassionate Targeted Case Managers (TCMs) to work with our team in Somerset, KY. As this is a Medicaid-supported service, TCMs will be paid per client. Rates. We are open to discussing this position as either a part-time or full-time position.

Duties

• The contractor agrees to maintain records of all services
• TCM is responsible for providing proof of a HIPAA compliant Telehealth platform that will be sent to HR for the employee files.
• TCM will use the ABA Sign Now for their electronic signature platform. TCM’s will sign off on ABA’s written policy for Electronic Signatures.
• Collects and uploads the consent packets, photos, checklists, and any Release of Information into EHR within 48 hours of initial contact.
• Completes written comprehensive needs assessment in collaboration with the child, his/her family, or other collaterals needed to determine the child’s needs.
• Needs Assessments are to be completed, signed and uploaded into EHR within two (2) weeks of the signed consents and will be updated annually or when significant life changes occur.
• Development of a treatment/service plan of care based on assessment date as well as a sound knowledge base of standards of professional practice and principles of their respected discipline. Service plans are to be completed, signed and uploaded into EHR within 30-days of signed consent and to be updated every 3 months (90 days).
• Documentation (i.e., Progress Notes) must be submitted no later than 8am on the 1st of the following month of services rendered. Progress notes must contain a full description of the service rendered. Progress notes turned in after the submission date may not be reviewed, signed, or paid out until the following month.
• Community Resources must be provided monthly and documented in the note with additional notes as to how/why this will benefit the client.
• Discharge summaries will be written, signed, and uploaded within 10 days of discharge.
• Monitoring the client’s progress with accessing needed services based on the assessment and the treatment interventions. ABA requires at least one (1) face-to-face (breathing the same air) each month. The other direct contact can be via HIPAA compliant telehealth.
• Coordinate supports with healthcare providers, schools, therapists, social services, medical equipment providers, and community resources as needed.
• Verify client insurance and eligibility monthly and update the EHR system accordingly.
• Must ensure Prior Authorizations are active and in the EHR system before billing service.
• Ensure New Intakes are not receiving TCM services elsewhere.
• Respond in a timely manner to any MCO Request for Information, Denials, or any other communication with the MCO regarding your client.
• Each staff person has a legal responsibility to report any form of abuse (physical, emotional, sexual), neglect or exploitation.
• Ensure the accuracy, content and completeness of client’s files.
• Respond to emergency situations in a timely fashion.
• Report directly to the Targeted Case Manager Supervisor(s) and/or Director.
• Additional duties as agreed upon by ABA, LLC and contractor.

Requirements

• TCMs must have a Bachelors in a Human Service field as per 907 KAR 15:060.
• TCM’s must turn in their CORE training plus at least one additional TCM training (SED, SMI, Co-Occurring) certificate(s) to HR for the employee file. If needed, SED will be required within the first three months of hire.
• TCM’s must meet the requirement of 6 CEUs per year and turn in the Crystal Report from the TRIS site each renewal period to HR.
• Per regulations, TCM’s must attend Group Supervision each month regardless of time acquired and/or experience.
• Per regulation, caseload caps at 25 clients per Targeted Case Manager and is the TCM’s responsibility to follow the rules and regulations if the contractor is under multiple contracts across agencies and not go over the 25-client cap.
• Prior Targeted Case Management Experience is preferred.

Pay: $28,000.00 - $116,700.00 per year
