TARGETED CASE MANAGER

*POSITION***SUMMARY:**Provides service coordination to targeted population clients (Child or Adult).

QUALIFICATIONS: Bachelor’s degree in psychology, social work, or mental health; preference for work experience with the targeted population assigned within the human service setting. Minimum one (1) year of experience post degree working with persons in a human services setting. Must provide own transportation to provide home visits.

SKILL SETS:Adequate communication skills, process verbal and written instructions, dependable, interact effectively with the public and staff, adaptability-able to deal with frequent change, delays or unexpected events, strong sense of commitment.

**PHYSICAL AND ENVIORMENTAL DEMANDS:**Visual acuity necessary for updating files and completing documents. Requires frequent stooping, kneeling and crouching to file records. Hearing and talking is necessary to answer phone and talk with employees and other individuals. Work is light with walking, standing and sitting. Manual dexterity is required to perform work processing and other clerical duties. Possible exposure to blood borne pathogens, biohazardous waste, hazardous waste and communicable diseases.

JOBSPECIFICFUNCTIONS:

1. Conducts home visits to assess service coordination needs for clients.

2. Coordinates and arranges for needed services as identified in treatment plan

3. Assists clients in accessing all needed services within the community

4. Performs advocacy on behalf of clients

5. Provides case consultations to service providers

6. Provide pre-tenancy and tenancy supports to assist participants in identifying, selecting, and entering into a lease agreement resulting in the participant moving into an independent housing unit.

7. Performs other duties as assigned.
