This is a full time, direct care position. The primary responsibilities of the Extender are to:

• Perform general outreach, engagement, and follow-up with members.
• Spanish language proficiency preferred.
• Coordinate services/appointments.
• Engage clients in health promotion activities.
• Share information with Care Manager on the member's circumstances.
• Make and track referrals.
• Participate in case conferences. Support the care manager in assessing and addressing unmet health-related resource needs.

Requirements:

• Must be a be at least 18 years of age, have a high school diploma (or equivalent)
• 1-2 years paid experience preferred within behavioral/mental health field.
• Spanish speaking/bilingual preferred.
