What they need

About Us

Next Chapter Healthcare provides support to individuals recovering from, and living with, mental health needs. Our supported living service is built around recovery-focused pathways, combining structured rehabilitation, meaningful daily activities, and community engagement.

We empower residents to build independence, confidence, and long‑term stability through personalised, person‑centred care informed by comprehensive occupational profiles that capture their goals and aspirations.

The Role

As a Team Leader, you will play a central role in ensuring high‑quality, consistent care across the service. You will support the day‑to‑day running of the home, supervise support staff, and ensure that residents receive coordinated, effective, recovery‑focused support.

This is a hands‑on leadership role, balancing team oversight with practical involvement in resident care and community‑based activities.

Key Responsibilities

Leadership & Service Coordination

  • Lead, support, and mentor a team of Support Workers to deliver high‑quality, person‑centred care.
  • Coordinate daily activities and rehabilitation sessions within the home and across the local community.
  • Ensure staff implement personalised care plans based on residents’ occupational profiles and recovery goals.
  • Monitor team performance, provide coaching, and support staff development.
  • Oversee safe staffing levels, daily scheduling, and task allocation.

Resident Support & Recovery Pathways

  • Promote a positive, empowering environment that encourages independence and recovery.
  • Support residents to engage in daily living tasks, community programmes, and meaningful activities.
  • Work collaboratively with nursing and occupational therapy teams to ensure care aligns with agreed pathways of support.
  • Act as a point of escalation for resident concerns, behavioural changes, or safeguarding issues.

Partnership & Community Engagement

  • Build strong relationships with local community charities and partners to support residents in developing independence and social networks.
  • Ensure community‑based activities are meaningful, well‑planned, and aligned with recovery objectives.

Quality, Compliance & Documentation

  • Ensure accurate, timely documentation in line with organisational and regulatory standards.
  • Support audits, care reviews, and performance monitoring within the service.
  • Maintain high standards of safety, dignity, and safeguarding across all resident interactions.

About You

We are looking for a Team Leader who is:

  • Passionate about mental health recovery, rehabilitation, and person‑centred care.
  • Able to lead, inspire, and support a team within a fast‑paced environment.
  • Skilled in building positive, trusting relationships with residents and colleagues.
  • Confident in decision‑making, problem‑solving, and managing day‑to‑day operations.
  • Motivated, reliable, and committed to maintaining high standards of care.
  • Experienced in mental health or social care (Team Leader/Senior Support Worker experience desirable).
  • Eager to participate in ongoing training and professional development.
