Job Summary

This role involves evaluating and assessing patients to establish functional, measurable goals aimed at restoring abilities. The provider develops and implements a treatment plan, monitors progress, and adjusts care as needed. Responsibilities include patient evaluation, care planning, treatment delivery, reassessment, and discharge planning with education and referrals. The role may involve supervising clinical students and support staff, fostering a caring environment that promotes healing. Additional duties include contributing to healthcare improvement initiatives and completing assigned objectives.

Key Responsibilities

• Assess patients based on subjective and objective data.
• Create and follow a written care plan.
• Deliver treatment aligned with care goals.
• Evaluate treatment effectiveness and reassess patient status.
• Provide discharge planning, education, and community referrals.
• Support staff and student development.
• Participate in quality improvement activities.
• Complete assigned tasks efficiently.

Qualifications & Skills

• Entry-level experience preferred.
• Basic skills necessary.
• BLS certification required within 14 days of hire.
