1 Transition Advisor for Sunderland College and/or Hartlepool Sixth Form

This is an exciting opportunity to join Education Partnership North East (EPNE) and help strengthen our SEND and inclusion transition offer across our college group. Working closely with secondary schools, caregivers, local authorities and college teams, you will act as the key link between young people and their future college, supporting early transition planning from Year 10 onwards and helping ensure that appropriate support is in place before learners begin their post-16 journey.

The role will contribute to the development of an inclusive transition model, supporting our commitment to improving outcomes for young people with additional needs. • Support young people and caregivers through college visits, taster sessions, transition events and meetings, helping them feel confident and prepared for the move into further education.
  • Work collaboratively with SEND Officers, Transition Advisers and curriculum teams to share relevant information appropriately and ensure suitable support is in place before learners enrol.
  • Maintain accurate records, monitor transition activity and outcomes, and contribute to reporting and improvement work to strengthen the effectiveness of EPNE’s SEND and inclusion provision.
