Transition Coordinator

Position Overview

We are seeking a Transition Care Coordinator to support members transitioning

between levels of care, including hospital discharge, community placement, and long-

term services.

This role focuses on care coordination, service linkage, and ensuring members receive

appropriate medical, behavioral, and social support services.

Key Responsibilities

 Coordinate transition care services for Medicaid members

 Conduct assessments and identify member needs

 Develop and implement transition care plans including risk mitigation plan

 Connect members to medical, behavioral health, and community resources

 Communicate with providers, case managers, and family members

 Monitor member progress and update care plans as needed

 Maintain accurate and timely documentation in compliance systems

 Support safe discharge planning and continuity of care

 Submit financial and community service program applications on behalf of

members.

Qualifications

 Bachelor’s degree in Social Work, Nursing, Public Health, Human Services, or a

related field OR 5+ years of experience working with Medicaid populations, care

coordination, case management, or social services

 Experience in care coordination, case management, or social services preferred.

 Knowledge of Medicaid and/or transition care services preferred

 Strong communication and organizational skills

 Ability to manage multiple cases independently

 Basic computer and documentation skills

 Valid driver’s license

 Ability to travel to member locations as needed

Preferred:

 Experience with elderly, disabled, or high-risk populations

 Familiarity with Colorado Medicaid or transition care programs

Work Schedule

 Monday to Friday

 Up to 40 hours per week (flexible scheduling)

Compensation

 $25/hour
