Transitional Care Coordinator

Follow patients through a post discharge window as they move from a hospital or an inpatient facility to home or another facility. Conduct timely phone calls within 2 business days of discharge from inpatient facility. Review and verify medication list. Completing transition of care questionnaire and scheduling patients for hospital follow up appointment within 7-14 days of discharge. Following all ED visits and making follow up appointment when needed. The goal of the transition of care coordinator is to reduce readmission, connect patients with their providers, and identifying patients needing ongoing support and health management.

Pay: $16.00 - $20.00 per hour

Expected hours: 35.0 – 40.0 per week
