Role: Transitional Care Nurse

Responsibilities

• Coordinate safe, patient-centered transitions from hospital to home or post-acute settings
• Lead discharge planning and risk assessments
• Reconcile medications and ensure proper patient education

• Prevent avoidable readmissions through comprehensive care and communication
