Job Summary

Provides support in planning, directing and coordinating care of patients. Assists in obtaining patient medical records, schedule patient diagnostic testing, contact patients by phone and/or written correspondence. Writes letters, mail and fax documents. Develops and maintains organized records and documentation.

Essential Functions

• Requests and obtains outside medical records from various providers and/or facilities
• Schedules initial and follow up appointments.
• Contact patients by phone, MyChart, written correspondence regarding appointments
• Faxes or mails medical information to case managers, referring physicians, dialysis units and consulting physicians
• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements

• Education - High School diploma or equivalent. Associate degree preferred
• Experience - Two (2) years related experience

• Advanced ability in use of electronic medical record, computer software, correspondence skills, maintenance of database
• Communication skills.
• Knowledge of medical terminology
