Transportation Coordinator

Five Horizons Health Services - Montgomery (FHHS-M) is a non-profit organization located in Montgomery, Alabama that strives to promote a healthy, compassionate, and educated community through medical care, prevention education, supportive services, advocacy, and research.

Overview:

The Transportation Coordinator is responsible for assessing and addressing transportation barriers, determining appropriate resources, and coordinating access to transportation services for clients of Five Horizons Health Services (FHHS). This position works collaboratively with Social Services and Clinical departments to ensure seamless coordination and provision of transportation-related resources.

Duties:

• Schedule, confirm, and directly transport clients to ensure they attend medical appointments at FHHS.
• Collaborate with Medical and Social Services staff to assess and prioritize the urgency of transit needs for clients.
• Supervise the Transportation Driver in safely operating company vehicles to transport clients to medical appointments and other designated locations, ensuring adherence to safety protocols and timely service delivery.
• Determine the most effective transportation resource and provide linkage to available services, ensuring timely access for clients.
• Maintain clean interior and exterior of vehicle(s) used for passenger transport and report any mechanical problems to the supervisor promptly.
• Ensure all clients have a current transportation waiver on file and comply with all traffic and safety regulations during transport.
• Maintain accurate client records, document services provided, and complete required paperwork in accordance with HIPAA, Ryan White, state guidelines, and FHHS policies.
• Perform any other department or agency-related duties or special projects as directed by the supervisor.
• During emergencies, be prepared to fill in at any position within the agency that aligns with the Transportation Coordinator's capabilities.
• Coordinate closely with Social Services and Clinical departments to assess ongoing transportation needs and develop strategies for improving service delivery.
• Serve as a liaison between FHHS and external transportation providers, fostering partnerships to enhance transportation options for clients.
• Participate in interdisciplinary team meetings to discuss client progress, transportation issues, and service coordination.
• Conduct regular assessments of transportation services to identify areas for improvement and recommend solutions.
• Ensure compliance with organizational policies and procedures related to transportation services and confidentiality.
• Manage vehicle maintenance schedules, ensure insurance coverage is current, and oversee any necessary repairs or inspections to maintain safe and reliable transportation services for FHHS clients.

Qualifications:

• Possess a valid Alabama Driver License
• Must be at least 21 years of age
• Must confirm effective auto-insurance coverage if utilizing a vehicle for work-related reasons
• No more than two (2) moving violations in the past three (3) years
• Ability to interact and provide respectful services to diverse groups and cultures

Job Type: Full-time

Pay: Up to $55,000.00 per year
