Treatment Coordinator

Project Turnabout | Willmar, MN

Project Turnabout is seeking a full-time Treatment Coordinator to join our Intensive Outpatient (IOP) team in Willmar, MN. This position works closely with our treatment team to coordinate services, connect patients with community resources, and support their recovery journey.

Key Responsibilities

• Assist Licensed Alcohol and Drug Counselors (LADCs) with coordinating aftercare services and assessing treatment availability.
• Facilitate referrals to substance use disorder treatment services based on patient assessments, provider recommendations, and treatment plans.
• Connect patients with community resources, including housing, economic assistance, social services, and prenatal care.
• Collaborate with the treatment team to support continuity of care and successful recovery outcomes.

Qualifications

• Bachelor's degree in Psychology, Sociology, or a related Human Services field required.
• Must be at least 18 years of age.
• Must successfully meet Minnesota Department of Human Services background study requirements.
• Must be able to document at least two years of freedom from chemical dependency or gambling-related problems.

Job Type: Full-time

Pay: $44,000.00 - $46,000.00 per year
