And we look after our team the same way. We offer real support, meaningful recognition, and rewards that reflect the difference you make every day.
Here’s what you can expect:
- Clear, supported career pathways and training to help you grow
- Full induction and buddy shifts

- Flexible and hybrid working options
- Employee referral reward program - because good people know good people
- Fitness Passport - access to gyms and wellness facilities
- Employee Assistance Program - confidential support when you need it
- QLeave - portable long service leave that recognises long-term care work
You may not be the one in the room, but you make sure the right support shows up to the front door.
As a Triage Scheduler, you'll play an important part in helping people access aged care services at the right time and in the right way.
Every referral represents a person looking for support. Your job is to review incoming referrals, understand what's needed, and make sure each person is guided to the most appropriate assessment pathway.
You'll work closely with assessors and internal teams, balancing workloads, keeping things moving, and helping make sure no one falls through the cracks.
This is a permanent full-time position working 76 hours per fortnight, Monday to Friday.
What you'll be doing
- Review incoming referrals and determine the most appropriate assessment pathway
- Assess eligibility for aged care services and escalate more complex matters when needed
- Allocate assessments across the team while maintaining balanced workloads
- Monitor progress and identify delays, bottlenecks, or resourcing challenges
- Maintain accurate records across systems and ensure compliance with policies and guidelines
- Work closely with internal teams to support timely, high-quality service delivery
- Use reporting tools and dashboards to monitor workflow and performance
- Identify opportunities to improve triage and allocation processes
- From time to time, support assessments or care plan reviews by phone, telehealth, or in person
You'll thrive in this role if you
You know how to stay calm when things get busy.
You can juggle moving parts, make thoughtful decisions, and communicate clearly, even when situations are complex or time-sensitive.
You'll also bring
- A tertiary qualification in a health-related discipline such as nursing, occupational therapy, physiotherapy or social work
- Current AHPRA registration or eligibility to join a relevant professional association
- A strong understanding of aged care pathways and the Aged Care Act
- Confidence using digital systems, including the My Aged Care portal
- The ability to work independently while staying connected to a team
- Flexibility and a practical approach to changing priorities
- Current First Aid and CPR certification
- A current driver's licence and willingness to use your own vehicle for work
- Experience working with people from different cultures and backgrounds, including vulnerable communities
Most importantly, you'll bring warmth, professionalism, and a genuine desire to help people live better, more connected lives.
