USRN Virtual Assistant

Pay: ₱35,000.00 per month

Job Description:

Conducts remote clinical assessments, monthly monitoring, and care coordination for participants in California’s Assisted Living Waiver (ALW) and California Community Transitions (CCT) programs. The goal is to monitor health status, ensure safety, and coordinate care to support stable community placement.

Key Responsibilities

Telehealth Assessments & Monitoring:

• Conduct initial, annual, and monthly nursing assessments via telephone or video.
• Evaluate health, functional status, medications, mobility, cognitive changes, fall risks, and environmental safety.
• Identify unmet needs or health declines and escalate for in-person follow-up, provider evaluation, or emergency care when necessary.

Care Coordination:

• Collaborate with participants, families, caregivers, physicians, assisted living facilities, and community agencies.
• Support transition planning and post-transition monitoring for CCT participants to prevent avoidable hospitalizations.

Documentation & Compliance:

• Timely and accurately document all findings, contacts, and coordination efforts in compliance with agency policies, program guidelines, and HIPAA standards.

Risk Management & Reporting:

• Monitor for health decline, safety hazards, caregiver burnout, or signs of abuse/neglect (acting as a mandated reporter).

Qualifications & Requirements

Licensure & Experience:

• Active California Registered Nurse (RN) license (minimum 1 year).
• At least 1 year of total nursing experience.
• Preferred: Experience in home health, hospice, case management, long-term care, or Medi-Cal waiver programs (ALW/CCT), as well as prior telehealth experience.

Key Skills:

• Strong clinical assessment, organizational, and written/verbal communication skills.
• Technical proficiency with EMR systems, email, and video call platforms.
• High level of independence, clinical judgment, and adherence to patient privacy standards.

Work Location & Schedule:

• Onsite Location: McKinley, Taguig
• Shift: US Hours (Night Shift)
