Virtual Patient Scheduler

About the role

This role involves handling phone inquiries, processing referrals, and managing patient admissions for home health services. You will communicate with liaisons and referral sources, review health records, create digital charts for new patients, verify insurance coverage, obtain authorizations, and coordinate with administrators regarding medication costs and insurance approvals.

Key responsibilities

• Handle phone inquiries and process referrals accordingly
• Communicate with the Liaison and referral source to notify them of referral receipt and provide timely updates on acceptance or denial
• Review health records prior to accepting referral
• Create a digital chart for the new patient
• Verify Primary Insurance coverage
• Obtain authorization by calling appropriate insurance provider if applicable
• Cost out medications and obtain approval from the Administrator if necessary
• Notify the Liaison when medication costs and insurance are approved

About you

• Bachelor of Science in Nursing (BSN) required
• Proficient in English language, both verbal and written
• Phones and customer service experience required
• Minimum of one (1) year experience with admissions in a Skilled Nursing Facility, hospital discharge, or case management
• Knowledgeable of medical practices, terminology, procedures, laws, regulations and guidelines pertaining to home health
• Background in working with Medicare regulations in Home Care
• Proficient word processing, keyboarding, and data entry computer skills required
• Experience navigating clinical information system networks required
• Able to relate positively, favorably and cooperatively with physicians, associates, patients, and personnel of outside agencies and organizations
