What they need
Southern Brooks is looking for a passionate and proactive individual to join our Welcome Home Project. Working in partnership with Age UK South Gloucestershire, you'll support people returning home from hospital, helping them regain confidence, maintain their independence, and stay connected to their local community.
This is a rewarding role where you'll work alongside people at a key point in their lives, providing tailored support that helps them overcome challenges and achieve the goals that matter most to them. You'll take a person-centred approach, empowering individuals to build on their strengths, access the right services, and feel confident living independently at home.
We're looking for someone who has experience supporting people with health, wellbeing, or social care needs and who is committed to making a positive difference in their community. You'll be a confident communicator, able to build trusted relationships with clients, colleagues, and partner organisations, and you'll be motivated by helping people improve their wellbeing and quality of life.
If you were already in this role, you might be doing some of these things this week:
- Carrying out person-centred home assessments and co-producing an action plan with clients.
- Supporting individuals to overcome barriers to maintaining their independence and accessing services.
- Accompanying individuals to community groups, activities, appointments, and statutory services.
- Supporting people to use assistive technology and practical solutions that help them stay safe and independent at home.
- Working closely with hospital discharge teams, occupational therapists, social care professionals, volunteers, and other partners to provide joined-up support.
To apply, you will need at least 2 years’ experience working with people with health and social needs, or community-based work using a person-centred approach.
As a vital member of our team and in return for your passion, expertise and commitment, we will provide you with:
- A salary of £28,088 per year (pro-rata)
- Excellent Annual Leave entitlement - starting at 5 weeks plus bank holidays per year (pro rata)
- Great sickness benefits
- A passionate, inclusive, committed and welcoming workplace
- A flexible, hybrid, casual-dress work environment
- And we will even give you a paid day off for your birthday!
Apply on our website to register your interest in this Welcome Home Team Worker position by 9am, Thursday 5th November 2026.
Interviews will be held on Wednesday 18th and Thursday 19th November 2026.
