We’re looking for a Care Coordinator Nurse who will help coordinate patient care, support chronic condition management, and ensure smooth transitions between healthcare settings.

Qualifications

• BS Nursing graduate with active PHRN license
• 1–3 years hospital/clinical experience, preferably in a tertiary hospital
• Case Management or Transition of Care experience is an advantage
• BPO experience is a plus, but not required
• Non-licensed BS Nursing graduates with healthcare BPO experience may also apply
• Strong communication and interpersonal skills
• Willing to work onsite in Bacolod and on a night shift schedule
