長者及健康服務
持有香港註冊護士證書及有效執業證書並具五年註冊護士經驗；
具五年或以上認知障礙症、長者及護老者服務經驗以及與不同專業協作經驗者將獲優先考慮；
負責統籌及協調各長者單位的護理服務運作及督導工作；
負責推行「智友醫社同行」計劃，負責評估服務使用者的需要及帶領小組工作；
支援社區患有輕度至中度認知障礙症的長者及其家屬；
定期與醫管局專業同工協調工作及進行個案會議；
工作地點：港島區。
