- 屬助理社會工作主任職級，註冊社工，持社會工作學位或以上學歷
- 具跨專業個案管理經驗、智障或肢體傷殘人士服務經驗優先考慮
- 負責個案管理工作、建立社區網絡、照顧者及家屬支援、籌辦活動等
- 負責管理及協調分區社工隊整體的行政、財政、人事、服務運作及方向，督導家居照顧員等工作
- 具良好組織及統籌能力
- 於九龍城及油尖旺/ 深水埗/ 西貢分區工作
