職責：

- 負責外展長者、有需要長者及護老者支援服務
- 負責長者中心中央資料庫及登記系統的運作，協調整合及更新數據工作
- 策劃及推行長者中心活動、小組、公眾教育及大型活動
- 協助推行機構中央事工
資歷：
- 持有認可社會工作學位或以上之註冊社工
- 具三年或以上安老服務工作經驗者優先
- 良好中、英文書寫能力
- 具「長者健康及家居護理評估員」資歷優先
