- 具長者服務、社區復康、家居安全評估或輔助器具 / 環境改善經驗優先
- 良好中英文書寫及溝通能力
- 能與社工、技術人員及承辦商協作
- 負責為長者進行家居安全及功能需要評估
- 就輔助器具、家居改裝及環境安全提供專業建議
- 制定簡單家居改善方案及跟進成效
- 指導技術主任及承辦商按專業建議執行改善工程
- 為同工及義工提供長者家居安全、跌倒預防及輔助器具應用培訓
- 協助建立評估工具、服務流程及個案紀錄標準
- 工作地點：深水埗區
