- 須具最少 3 年相關副學士 / 文憑或以上後經驗
- 具長者服務、社區工作或外展服務經驗優先
- 有耐性及良好溝通能力，能與長者、家屬、義工及地區人士合作
- 良好中文書寫能力及基本電腦應用能力
- 需於週末、公眾假期及晚上輪班工作
- 協助進行外展接觸、家訪及初步需要評估
- 跟進一般個案、活動及小組服務
- 協助長者連繫社區資源及支援網絡
- 支援義工探訪、社區宣傳及大廈接觸工作
- 整理個案紀錄、活動紀錄及服務統計
- 工作地點：深水埗區
